The FMEA training will be focused mainly on PFMEA analysis, taking into account the basic elements of DFMEA. Participants will become familiar with the FMEA methodology through practical examples, with an emphasis on process analysis, but also with reference to design analysis – in order to understand the relationships between DFMEA and PFMEA and to be able to apply them in practice.
As part of the workshops, participants will acquire skills in defining nonconformities, describing prevention and controls, and planning actions to reduce risk.

TRAINING PROGRAM

For in-company training, the proposed program is a preliminary suggestion and can be freely adapted to the individual needs of the company.

The following program is indicative (the division into days is arbitrary) and the trainer adapts the course of the training to the specific characteristics of the group. Individual topics may be discussed at a different time or in a different form than indicated in the plan.

Module 1

BASICS OF DESIGN AND PROCESS FMEA

Content:

  • FMEA as a risk management tool – what improvement of designs and processes is about
  • “Failure Mode” – different approaches to defining nonconformities
  • “Effect Analysis” – customer groups for effect analysis
  • FMEA analyses in different project phases: design, pilot series, serial production (APQP and MLA)
  • 8D and FMEA – necessary common points (ensuring documentation compatibility)
  • FMEA effectiveness – FMEA analysis and the cost of quality calculation

Module 2

RISK ASSESSMENT PRINCIPLES IN FMEA

Content:

  • Design and process FMEA – outline of the analysis flow
  • 7 steps of FMEA analysis according to AIAG & VDA requirements
  • Links between FMEA and related risk assessment methods
  • FMEA forms – overview of possible options
  • Risk assessment: AP, SOD, RPN, risk matrix

Exercises:

Analysis of FMEA results – risk assessment using multiple indicators
Defining the risk level using AP, RPN, and the risk matrix
Process and product characteristics in FMEA – case study

PLANNING AND BUILDING STRUCTURES FOR ANALYSIS

“5T” – new approach to FMEA planning

  • STEP 2. PFMEA: Process structure analysis:

defining process steps and work elements (4M+)

boundary diagram (BD)
visualization of the product structure

  • STEP 3. Function analysis:

functions at the customer level – expectation analysis
functions as expectations in the process or design

  • STEP 4. Failure network analysis:

defining nonconformities

determining the effects of nonconformities for all customer groups

RISK ASSESSMENT AND PLANNING OF IMPROVEMENT ACTIONS

Content:

  • STEP 5. Risk analysis:

new and “old” S, O, D rating tables – comparison
prevention vs detection and their impact on the risk rating value
Action Priority (AP) – successor to the RPN indicator (determination and approach to improvement)
actions in the case of AP=H and AP=M – when it is not necessary to implement actions

  • STEP 6. Optimization:

changes in documenting planned actions, suggested statuses
methods for evaluating action effectiveness
describing the expected risk level

  • STEP 7. Reporting:

FMEA report – new mandatory document
FMEA revisions – when mandatory and how to document them
sharing FMEA outside the organization – new guidelines
Changes in FMEA as a result of internal and external complaints

ADDITIONAL INFORMATION

The training has been designed based on the new FMEA analysis handbook.

The training program is highly universal, as it is based on the separate requirements of AIAG and VDA while at the same time constituting an up-to-date presentation of the new joint standard.

After the training, each participant will receive a certificate confirming completion of the training.

What is FMEA?

FMEA (Failure Mode and Effects Analysis), or Failure Mode and Effects Analysis, is a method aimed at identifying, assessing, and minimizing the risk of failures, defects, or errors in a product, process, or system. This analysis enables early detection of potential problems and the implementation of appropriate improvement actions.

FMEA is widely used in industries such as automotive, aviation, the electronics industry, medical device manufacturing, and many others, where quality and reliability are of key importance.

Participation in the FMEA course allows for a better understanding of the principles of applying this method in practice and for organizing the approach to risk analysis.
