TRAINING PROGRAM

For in-company training, the proposed program is a preliminary suggestion and can be freely adapted to the individual needs of the company.

The following program is indicative (the division into days is arbitrary) and the trainer adapts the course of the training to the specific characteristics of the group. Individual topics may be discussed at a different time or in a different form than indicated in the plan.

Module 1

INTRODUCTION TO THE 5 WHY METHOD AND ITS APPLICATION

Content:

  • Origin of the 5 Why method – A brief history and background of this problem root cause analysis tool
  • Connection with other quality tools – How the 5 Why method integrates with approaches such as 5W2H (What, Where, When, Who, Why, How, How much) and FMEA (Failure Mode and Effects Analysis)
  • The role of the team in analysis – The importance of the moderator and the involvement of the entire team in the cause identification process
  • Three aspects of analysis – An overview of how the 5 Why method supports the analysis of causes related to occurrence, detection, and systemic failures

Exercises:

A short case study analysis aimed at identifying initial problems and their context

A panel discussion on roles within the analytical team

Module 2

RACTICAL APPLICATION OF THE 5 WHY METHOD

Content:

  • What a root cause is – A detailed explanation of the definition of a root cause and how to correctly identify it
  • Principles and best practices – A step-by-step guide on how to conduct a 5 Why analysis effectively
  • Validation and evidence – How to verify identified causes using concrete evidence and data
  • Comparison with the Ishikawa Diagram – A discussion of the differences, strengths, and limitations of both methods, as well as their complementary use

Exercises:

Practical analysis based on selected examples – participants work in groups using the 5 Why method to solve real or simulated problems

An exercise focused on validating causes and collecting supporting evidence

Module 3

PITFALLS AND BEST PRACTICES IN ANALYSIS

Content:

  • Strengths and limitations of the 5 Why method – A critical assessment of the method, including its benefits and constraints
  • Pitfalls and common mistakes – The most frequent errors made when applying the 5 Why method and how to avoid them, such as focusing on symptoms rather than root causes
  • Examples of application across different fields – Illustrations of how the 5 Why method is used in various industries and areas, such as manufacturing, IT, and services

Exercises:

Analysis and improvement of incorrect or incomplete 5 Why examples

A discussion on when it is more appropriate to use the 5 Why method and when other tools may be more effective

ADDITIONAL INFORMATION

The training is delivered in a workshop format, supported by a multimedia presentation and numerous practical exercises. The theoretical part is reinforced with multiple examples that help participants better understand the certification guidelines.

After the training, each participant will receive a certificate confirming completion of the course.
